You are in Control

You control your personal health information and can determine what information is made available to Emergency Medical Personnel in the event of an emergency. You are the only user who can edit your personal health information. Portrait of your Health will not sell, rent, or share your information without your explicit consent, except in such situations, as required by law. You can completely delete your personal health information at any time. Such deletions will take immediate effect in your account. However, backup copies of deleted information may exist for a short period of time. Since deleted data will not be restored, you may want to print information for your own records before deleting it.

View only record access

Each member can create a “view only” emergency page. In the event of a medical crisis, access is only available if the emergency personnel are in possession of your Emergency Medical Data Retrieval Card. If in the event you lose your card, simply call our toll free number to receive a new card and account number. This will disable your lost or stolen card. In such cases, all information in your personal health record is retained but blocked until you reset your password.

Your Portrait of your Health password

Choosing a password is your responsibility. Your password helps you keep your Portrait of your Health information private and secure. Here are some important things to remember about your password:

• Use a password that is difficult for someone to guess but easy for you to remember.
• Your password should be at least five characters long.
• Use a combination of letters and numbers.
• Don't use your birthday, a family name, a pet name, or other information that might be       easy for someone to guess.
• Do not tell anyone your password.
• Remember to log out when you leave any personal computer.
